Where to go after getting your photos from Jon's Darkroom:

If you are renewing an existing passport that has not expired you can just send in the renewal form in the mail. These forms can be downloaded by clicking the link further down this page. If you are applying for a new passport you can go to the courthouse on Main Street in Burlington, but our customers have found it more convenient to go to any of the following town clerk's offices:

What to bring with you:

You will need to bring the following documents with you:
-2 passport photos provided by Jon's Darkroom
-Completed passport application form (You can download the form you need by clicking the link below.)
-Certified (raised seal) birth certificate
-Photo ID (driver's license)
-Old passport if you have one
-Marriage license if name has changed
-Check for U.S. Dept. of State
-Cash or check for Town Clerk's Office

Cost of aquiring your passport:

If 16 years or older, passport is good for ten years and applicant will need to write a check for $67.00 to the U.S. Dept. of State, and pay the Town Clerk's Office $30.00 in either cash or check.
If younger than 16 years, passport is good for five years and applicant will need to write a check for $52.00 to the U.S. Dept. of State, and pay the Town Clerk's Office $30.00 in either cash or check.
You can expedite your order for an additional $60.00 plus postage. You will receive your passport in the mail in 3-4 weeks instead of the 8-10 weeks for regular applications.
